As the largest community healthcare NHS Trust in London, we provide a wide range of local and community-based healthcare services to nearly one million people in the locations we serve.

Community healthcare has a number of benefits. Most importantly, treating patients in their homes and in the communities where they are comfortable helps us provide a more holistic, well-rounded and bespoke healthcare service to each and every one of our patients. Improvements in treatments and technology mean more people than ever before can be

effectively treated in their own home. Not only does this provide faster and more convenient care for our patients, it also comes at a lower cost to the NHS.

Our services range from district nursing, health visiting and home-based rehabilitation programmes, to NHS walk-in centres, specialist diabetes services and educational nutrition and dietetics sessions.
