Birth & Death Certificates

TheCity of Cincinnati issues Birth Certificates for anyone born in Ohio. Certificates are public records and may be obtained by providing us with the necessary search information. Birth records are available from 1909 to present. All certified copies have a raised, embossed seal of the State of Ohio.

Birth Certificates are now applied for by using Cincinnati Vital Records Kiosk. Enter the correct information. Pay for certificate at the Kiosk. Customers may pay for their certificates via cash or credit/ debit cards. Vouchers are not accepted at the Kiosk.

  • Birth Certificates: $22.00 per copy
  • Death Certificates: $27.00 per copy.
  • There is a $3.00 Non-refundable search fee on records not found

Please have the following information available when placing

orders

  • Full name as listed on the birth certificate
  • Date of birth
  • Mother's name and maiden name (her name prior to first marriage)

The City of Cincinnati Vital records has added three Satellite kiosks for more convenience to order Birth or Death certificates. These hours and locations are listed below. Death certificates may be ordered at Kiosk locations, but will be mailed out the next business day with the exception of our Main office at 1525 Elm Street. Same day service is available.
