All Georgia County Birth Records can be obtained from Fulton County Vital Records Office. 99 Jesse Hill Jr. Drive, Room 101, in downtown Atlanta. The office is open from 8:30 A.M. until 4:30 P.M. (E.S.T.), Monday through Friday, excluding major holidays. Click here for the driving directions. Applications are available in our office and must be filled out in duplicate.
On-line applications can be placed through Vital Chek at www.vitalchek.com 24 hours a day, seven days a week.
Any birth record that Fulton County is unable to retrieve can be obtained by the State of Georgia Vital Records Department: 2600 Skyland Drive, Atlanta, Georgia 30319. Telephone (404) 679-4710.
The Vital Records Office issues Death Certificates for Fulton County only (deaths that occur in Fulton County only ).
Identification ("ID") is required to pick up certified copies of vital records in person. No copies of ID or expired ID will be accepted. The acceptable forms of identification are listed below.
Requires ONE of the following:
- Government-issued employee I.D. badge with photo
- Driver's license, any state
- State issued I.D. card
- Social Security Card
- U.S. Passport
Note: Copies of expired ID's are not accepted
Requires TWO of the following:
- WIC Voucher
- Monthly Medicaid Card
- Insurance card/policies
- Georgia utility bills
- Credit Card
- Payroll Check Stub
- W-2 Form
- Completed Income Tax Return
- Military ID
- Automobile title/registration
- Grady Hospital Card
- Library card
All forms of identification must show your current name and address. Expired cards or documents will not be accepted.
You have to send a photo copy of your present identification if the certificate copies are mailed to you. There is no additional charge for postage.
The cost for one copy of the birth certificate is $25.00 ( if record is found). Additional copies of the same record, purchased at the same time as the first, are $5.00 each. This cost is for a certified copy of the birth or death certificate. Acceptable methods of payment for in-person requests are cash, credit/Debit (Visa or MasterCard), money orders and personal check.
