Eligibility

  • If the identification you are submitting has your information on the front and your signature on the back, you must send a copy of both sides. Please make sure the copy is readable. Enlarging and lightening the image of your identification on the copier may help.

Required Information

  • Name as listed on birth certificate
  • Date of birth
  • City or town of birth/County of birth
  • Name of hospital where birth occurred (if known)
  • Father’s full name, if adopted, list adoptive father’s name.
  • Mother’s full maiden name, if adopted, list adoptive mother’s maiden name.
  • Birth certificate number (if known)
  • Local registration number (if known)
  • Purpose for which the record is required
  • Relationship to person whose record is required
  • If attorney, give name and relationship of your client to person whose record is required:
  • A photocopy of the current government-issued photo id of the person making the application, i.e. current driver's license
  • Name and address of the applicant
  • Telephone number

Procedure

In Person

  1. Print a request form: Birth Certificate Request Form .
  2. Fill out and complete the request form.
  3. Go to the Office of Vital Statistics.
  4. Present a signed form of identification at the time you place the order.
  5. Pay the fee.

By Mail

  1. Print a request form: Birth Certificate Request Form .
  2. Complete and sign the form. Be sure you list your name at birth and your mother’s maiden name – not her married name. Incomplete applications may cause a delay in processing.
  3. Include a check or money order payable to the Office of Vital Statistics, or a signed credit card form.
  4. Include a signed copy of your identification.
