What should my marriage certificate say as far as names?

I was just married a little less than a month ago and I am just now getting around to changing my name, only I've stalled for one reason: When I went to pick up my certified copy of my marriage license, it reads: Bride Name: first: Kandice middle: Angeline last: A**** maiden: A***** Basically, it says my. show more I was just married a little less than a month ago and I am just now getting around to changing my name, only I've stalled for one reason: When I went to pick up my certified copy of my marriage license, it reads:

Bride Name:

first: Kandice middle: Angeline last: A**** maiden: A*****

Basically, it says my last and maiden name are still the same and nowhere does it say that I am Kandice M**** [my new name]

Do they do that just to say who I was

before we got married or was there a mistake made?

And before anyone asks:

I DID sign my marriage license as my new name.

When you look at where it says last name, you can tell it was typed in by a typewriter, it is not at all similar to the clearly computer printed text the rest of the document is in.

I can supply of picture if anyone is still confused

Update: I understand this doesn't change my name, I know I am to be the one to change my SS and driver license, ID and allthat jazz. I was just wondering if that was correct for my last and maiden name to be stated on here as the name i was given at birth.

Update 2: Oh and, when we filed for this "application and record" I signed it Kandice A MaidenName.
