Which immunization records are available?

In general, records may be available for people immunized in the City of San Antonio and Bexar County IF records have been provided to the San Antonio Metro Health District by

  • City Clinics
  • Personal physicians who make their immunization records public

Immunization records will not be available for immunizations provided outside of Bexar County or by personal physicians who do not make their immunization records public.

What is the cost of an immunization record?

The cost is $5, paid by cash, check, or money order to the City of San Antonio. Branch Libraries will also accept credit/debit card payments for birth certificates and immunization records. The fee for a returned check is $30.

How do I request immunization records?

At one of the participating branch libraries, check in at the Circulation Desk. Complete a immunization record application form. Bring the completed form back to the Circulation Desk and show your identification (all forms of identification must be original). When your record is ready, staff will call you by name (usually within 20 minutes). Return to the Circulation Desk to pay and pick up the immunization record.

Who can obtain an immunization record?

  • The patient (must be over 18 years old, emancipated, or have a valid driver's license)
  • The minor patient’s parent
  • Due to recent (May 2015) increase in security for these records, any other person wanting to obtain a patient's record must bring their ID, a copy of above's ID, and a permission form (only located there) and go to:
